ValueCare develops an application to improve the social and health care of elderly people with a Personalised Care Plan

Within the framework of the European ValueCare project, which aims to improve the social and health care received by frail people over 65 years of age, a digital application has been developed that synchronises data from multiple sources (health and social). This application will provide older people participating in the project with a Personalised Care Plan that combines medical and social prescriptions according to their interests and social and health needs.

In this sense, and after having recruited the elderly people who will be the participants in this pilot, the training has started today, presenting the application and training the participants in its use.
